Kutaj (Holarrhena) is a shrub or small tree. Its principal constituent is the alkaloid of kurchi that is conessine. The other alkaloids reported to be there in the bark are: conamine, conkurchine, connessimine, kurchine, conarrhinine, holarrhinene, and isoconcessimine. The bark has astringent, anti-dysenteric, anthelmintic, stomachic, febrifugal and tonic properties. It is used in the treatment of amebic dysentery and diarrhea.
In ancient Ayurvedic texts (Samhitas and Nighantus) Kutaj is used for the treatment of diarrhea, dysentery and acute piles.
In Charka samhita it is prescribed alone or with other ingredients in different weight and doses in many diseases like skin problems, piles, itching, fever, urine related problems, diarrhea, tuberculosis, dysentery, intestinal problems, heart related problems, wounds and vomiting purpose. Maharishi Sushruta prescribes Kutaj in urinary problems, including diarrhea, Pravahika, Udavasta, heart related problems, Anaha, piles etc.
Maharishi Vagbhata following Charka states its application for the treatment of Mutraghat. Shrangadhara has mentioned some kutaj preparations for the treatment of acute and chronic Atisara, Raktatisara, Jwasa, Pravahika, Pnadu, Raktapitta, Kamala, Amlapitta and Shotha etc.
Bhavprakash nigantu (BPN) has mentioned Kutaj as a remedy for Shukrashmaree (urino-genital disorder), when administered with curd. The diseases like Dysuria, Uterine bleeding and retained plachita are also treated with Kutaj. Yoga-ratnakara and Bhaishajya-ratavali, have followed the same concept the seeds of Kutaj have also a significant importance in therapeutic purpose. The seeds of Kutaj are mentioned in various literatures by the name Indrayaba, Kutajaveeja, Kutajaphala, Kalingayona and Vatsaveeja. It is administered for the treatment of Kapha humor, loss of appetite, heart related problems, dysuria, skin problems, malaria, epitasis, leucorrhea, goiter, piles, cholera etc.
This herb is a well known drug for amoebic dysentery and other gastric disorders. A clinical trial records the presentation of 40 cases with amochiasis and giardiasis. The efficacy of kutaj in intestinal amochiasis was 70 percent. Good response was also observed in Entamoeba histolytica cyst-passers when treated with kutaj bark. The flowers improve appetite. Holarrhena antidysenterica, Apocyanaceae, well known for its anti-diarrhoeal activity was studied for its effect on diarrhoeagenic Escherichia coli. Different dilutions of the decoction of the plant were assayed for its effect on the adherence and toxin production of two groups of Escherichia coli. The decoction was more successful in inhibiting stable toxin production in contrast with labile toxin production.
Research - Quantitative estimation of Holarrhena antidysenterica bark total alkaloids in crude drugs and in the body fluids of man and rat.

The research was carried out at Department of Physiology, Banaras Hindu University (BHU), Varanasi, India.
A turbidimetric method was developed for the quantitative estimation of the total alkaloids of kutaj barks i.e. Holarrhena antidysenterica or kutaj in crude medicinal preparations and in the body fluids of man and rat. The alkaloids were colloidally precipitated with Dragendorff's reagent as complex salts of potassium iodo-bismuthate in extremely dilute solutions. The gauzily subdivided orange-brown precipitate gave a colored, clear homogeneous suspension in the presence of gum Arabic. Optical density of such suspensions changed linearly with the change in alkaloid concentration, when arranged within the standardized experimental conditions that includes control of ion concentration and temperature of the reaction mixture. Observations bared the reversible nature of the alkaloid-reagent reaction. Crude curative preparations from 3 different pharmaceutical sources contained changeable concentrations of the alkaloids. Complete revival of the alkaloids was possible from plasma and urine, while important amounts of the alkaloids were lost to blood cells and faecal contents in man and rat
